POSITION SUMMARY

The Community Health worker will provide outreach, engagement and education about substance use disorders (SUD) and services available, foster community connections and increase access to services . The Community Health Worker engages the community in the services and activities of the Health & Wellness Center and provides education, outreach and support to the community as well as individuals and families served. The Community Health Worker works within the multi-disciplinary team to reduce disparities to care, supports individuals and families with substance use disorders, increase access to primary care, behavioral health and community resources. Wheeler Health & Wellness provides an integrated, comprehensive continuum of patient-centered primary care, behavioral health and community resources to enhance health equity and achieve the triple aim of health reform: improve health, reduce costs & increase quality. May function as a member of the Mobile Medical Team and provide services on Wheeler’s Mobile Medical Unit(s).

Hourly Rate: $18

Bilingual Stipend Available

EDUCATION AND EXPERIENCE/QUALIFICATIONS

High School Education or GED plus 2 years of experience working with individuals and families in the community are required.

Knowledge of or residence in the target community of New Britain or Waterbury, strongly preferred.

Valid Drivers License is required. Safely operates vehicle in the community, based on program needs. Must have held driver’s license for a minimum of one year.

Ability to work on a team to ensure program goals are met, communicate effectively verbally and in writing, and is committed to personal and program growth. Ability to manage time effectively. Ability to represent the Clinic in a professional manner.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    Provides cultural mediation and eliminate barriers to accessing the New Britain Medication Assisted Treatment Program. Create linkages in the community to help improve engagement, retention, access to care, and health outcomes for the population of focus.

    Conducts and participates in culturally appropriate community outreach and engagement activities

    Supports individuals and/or families in the education of substance use disorders and facilitate entry into Wheeler Health and Wellness MAT program and integrated services.

    Connects individuals to community resources to address social determinants of health

    Supports care management services and supports the development of care management plans specific to each assigned client and family, including interventions necessary to increase the probability of treatment completion, and to improve life functioning.

    Maintains records of coordination of care, outreach, patient support and/or care management activities for reporting and tracking purposes and completes all documentation for client and program records in accordance with documentation timelines and in compliance with Medicaid, licensing and other regulatory requirements/agencies as defined by program.

    Participates in the planning, coordination, and implementation of community engagement, education, outreach and support activities designed to enhance access to primary and behavioral health care. Outreach and education activities include prevention, health promotion, and wellness training events and programs; health screening events; recovery support services; mutual support groups; and referrals to other community resources.

    Provides peer support and role modeling in the office and/or community settings outside of the office to teach individuals and/or families how to identify needed resources and supports in their communities.

    Supports daily program specific operational functions as required. May transport clients to appointments or linkages as appropriate. May involve visiting clients in their homes and/or community settings

    Attends and is prepared for scheduled supervision, team meetings, staff meetings or rounds. Seeks additional supervision or consultation as needed and follows through with supervisory directives.

    Responsible for safely driving and operating the Mobile Medical Unit, including all associated operations and maintenance tasks. Tasks may include, but are not limited to operating and/or maintaining the unit’s slide, leveling system, and gas.

    Demonstrates clear, concise, professional writing and oral reporting skills with basic computer skills.

    Works effectively as a member of a multi-disciplinary team by communicating appropriately and positively with colleagues, families, internal and external service providers, administrative staff and actively preparing for and participating in team meetings.

    Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
